ACH Dispute Form
This form must be completed and signed to dispute an unauthorized electronic transaction on your account. (Not for debit card or credit card disputes)  

Address Change Form
If you need to update your address we will need this form completed, signed and returned to us.

ATM Fraud Dispute Form
For fraudulent ATM withdrawals in which a debit card/ATM card has been used.  For all other types of debit card fraud/disputes, please call 1-800-808-6402 to speak with our dispute department. 
 
If you wish to close your account, please complete and sign this form.

Debit/ATM Card Application
Debit card is free/ATM card is $10.00

EFCU Account Card
Must be completed along with the IRS W-9 to start your application for membership 
Must be completed for any changes to an account
