Below are various forms, which may be filled out, signed and securely submitted back to us online. If you need to make changes to your account (such as adding a product or changing a name) please call us at 303-428-5080 or email us at memberservices@electricalfcu.org.

Account Closure Form

If you wish to close your account, please complete and sign this form. After the form is received, you will be sent an email requesting a secure upload of your government issued ID.

ATM/Debit Card Form

Our debit card is free, and our ATM card is $10.00. If your card has been lost or stolen and you need a replacement card, please contact us. If outside of business hours, call 1.800.528.2273 to cancel the card.

Courtesy Pay Request Form

An overdraft occurs when the available balance in your account isn’t enough to cover a transaction, but as a service to you we pay it anyway. This form explains our standard overdraft practice and allows you to opt in to Courtesy Pay.

ACH Dispute Form

This form must be completed and signed to dispute an unauthorized electronic transaction on your savings or checking account. (For debit card disputes, click here.)

Automatic Loan Payment

Set up your loan payment to automatically transfer from another financial institution. You can also set this up from your online banking account.

Holiday Skip A Pay

Skip your November and/or December Loan Payments. There is a $30.00 fee per month per loan being skipped. Rules and restrictions apply. Only valid on auto and signature loans.

Address Change Form

Complete this form to update your address. After the form is received, you will be sent an email requesting a secure upload of your government issued ID. (You can also sign into your home banking account to request an address change).

Checking Closure Form

If you wish to close only your share draft/checking account, please complete and sign this form. (Your shares/savings remains open).

Round Up Savings Form

For EFCU’s Round Up Savings Program. Enroll and watch your savings account fill up by simply using your debit card.
